The Collaborative Care Model helps health systems meet their patients’ behavioral health needs without leaving their medical home.

CoCM adds two new members to the treatment team—a behavioral health care manager (BHCM) and a psychiatric consultant—using each of their unique strengths to optimize patient care.

Collaborative Care isn’t just a great model of care—it’s a great investment, too. Health systems that implement CoCM see a 6:1 return on their investment. Cost savings are gained across the emergency department, specialty mental health, medical and surgical services, as well as in pharmaceutical, inpatient and outpatient costs.
